this is at least good news in so far as it seems to exclude
one possible cause of the problem, thus bringing you one
step closer to the solution.

Can you absolutely rule out any caching proxy sitting between
your server and the browser in question?
If you have tracked down such a browser, can you then send
some requests from this browser to your server and trace the
requests inside your access_log, so that you can be sure that
it is really your server sending these broken images, and not
some cache somewhere in between?
